In cases of tibial insufficiency or traumatic/ infected bone loss involving tibia , the fibula is osteotomized and fixed to tibia to convert it into the main weight bearing bone. This is called tibilalization of fibula or medialization of fibula. 
It is done in two stages. Osteotomy is done at proximal end, then at lower end and fixed to tibia with screws and cancellous graft. Weight bearing is avoided for 8 weeks and once union is achieved, patient is mobilized. Gradually the fibula increases in girth and strength and takes the role of tibia. This was first popularized by Huntington. Hence called HUNTINGTON's procedure.
